This easy-to-use form tells
us what to inscribe and/or where to send up to ten personalized porcelain ornaments.
You can receive them all at the same address or ship them to different addresses. (USA
only.) If you are shipping to different addresses, you must place separate
orders. You can include the inscription or gift tag information in the
Comments" area of our shopping cart. If you have
more than five recipients or a custom name

adding the names, addresses and six word message that you want to send to
each recipient.

If you forgot to enter a
special discount offer code on the Order Form, it's not too late! Enter it
in the "Comments" section below and we will adjust your order.
